Complete the COVID-19 Notification Form
For the well-being of our entire Seminary community, please notify the Seminary immediately if you are experiencing flu-like symptoms; are being tested for COVID-19; or have been in close contact* with those who are experiencing symptoms, are under self-isolation, or are being tested themselves.
*According to the New Jersey Department of Health, a close contact is defined as: (a) being within approximately six feet of a COVID‐19 case for a prolonged period of time; close contact can occur while caring for, living with, visiting, or sharing a health care waiting area or room with a COVID‐19 case; or (b) having direct contact with infectious secretions of a COVID‐19 case (e.g., being coughed on).
Complete the Travel Notification Form
Following CDC guidelines, we recommend limiting all non-essential travel. If you must travel, it is your responsibility to follow New Jersey guidelines and self-quarantine upon return from travel to states with significant spread of COVID-19, in accord with the New Jersey travel advisory. Please check the state website, as the list of impacted states is updated regularly. To notify the Seminary, please complete the Travel Notification Form.
Complete the Vaccination Form
We request that all students, student spouses, and employees who are completely vaccinated please submit a copy of your vaccination record to the health team through the Vaccination Form. All information will be kept confidential.
